Social Prescribing Coordinator

Job Description

This is an exciting opportunity to join Impetus, a leading loneliness charity, working to improve wellbeing and reduce social isolation through creating community connections.

We are looking for a dynamic and values-driven person with outstanding interpersonal skills and a desire to help individuals achieve their fullest potential through holistic support. You will have proven experience of developing person-centred plans with service users and a track record of supporting and overseeing the work of volunteers.

Excellent record keeping skills, a commitment to equality and diversity and an ability to thrive in a fast paced and changing work environment are essential.

Hours

18.5 hours a week, worked flexibly, including occasional evenings and weekends

Salary

Based 01/04/19 NJC scale points 19-22 (£24,799 – £26,317 full-time) pro-rata, + 3% employer’s contribution to auto-enrolment pension

We are looking for a dynamic and driven person with exceptional communication skills and a strong collaborative ethos to join our cutting edge Social Prescribing service. You will have proven experience of developing and implementing communication plans to optimise successful service delivery and will thrive in a fast-paced and evolving service environment.

Excellent planning and monitoring skills and a commitment to equality and diversity are also essential.
